Song at the end of Gilmore Girls?

On the Gilmore Girls DVDs when they run the credits and you hear a guitar and then drums playing, what is the guitar chords? I would really like to play it. Does anyone know who worte it, or if there is a website I can go to, to find the music or know the chords? Please anwser!
